Do you use KoboToolbox, ODK, or XLSForms to collect demographic data in your programme? Data such as age, gender, disabilities, or income? I'm creating a package of free templates you can download and use for collecting this type of data. So far, I've created a training video on collecting age data, and you can download the template for collecting age data here on this page. Age Data: There are four basic ways to collect age data when you’re doing an assessment or survey.
Have someone select the age category they fall into.
Ask someone their age, and select that age directly.
Ask someone the year they were born, and then calculate their age.
Ask someone their exact birthday, including the day, month, and year.

Disability Data: One of the easiest ways to disaggregate your programme impact results by disability is to use the Washington Short Set of Questions on Disability. In the XLSForm template, these questions are used to collect data about health problems with:
1. seeing
2. hearing
3. walking
4. remembering
5. self-care
6. communicating
